|
TITLE: Senior Case Manager
OFFICE: Brooklyn EOC Bridge Program
LOCATION: Brooklyn, New York
SALARY: $55,000 - $60,000
FUNCTION & SCOPE: Under the supervision of the Bridge Administrator the incumbent will provide case
management and counseling.

| JOB REQUIREMENTS |
|
SPECIAL SKILLS: Must possess a strong commitment to social welfare and possess the necessary assessment skills to evaluate participant’s needs and problems. Ability to effectively develop and implement participant individual action plans. Must have excellent administrative, interpersonal, oral and written communication skills. Ability to exercise independent judgment as needed. Working knowledge of Microsoft Office Suite preferred. Bi-lingual Spanish/Haitian is a plus.
EDUCATION AND EXPERIENCE: MSW preferred, or Master’s degree in counseling and four years work experience in supervision,counseling, case management and crisis intervention preferably working with TANF and economically disadvantaged populations. Experience conducting workshops and presentations. Demonstrated experience in creating and maintaining successful working partnerships/collaborations. Routine local travel required.

|
TITLE: Senior Case Manager

FUNCTION & SCOPE: Under the supervision of the Bridge Administrator
DUTIES:
•Provide on-going case management and counseling to assigned participants.
•Work with instructors and other staff to support participant educational and employment goals.
•Work with participants, identified by the instructors, to address factors that are impacting program participation and attendance.
•Assist employed participants, both on-site and off-site, in overcoming barriers that may affect employment retention.
•Coordinate/supervise the administration of appropriate participant assessment and testing and ensure that participant case management records and notes are maintained and up-to-date.
•Provide referrals to other appropriate community partners for programs and services not provided by the BEOC Bridge Program.
•Conduct follow-up with other community partners providing programs and services to assigned participants to ensure participant needs and goals are met.
•Supervise the design and delivery of orientation and life skills workshops and other appropriate workshops.
•Cultivate and maintain partnerships with community agencies to provide participants with a continuum of supportive pre-employment and post-employment programs and services.
•Develop, implement, coordinate, and evaluate agency-wide Bridge recruitment/marketing plan
•Assist Bridge Administrator in the supervision of other case management staff.
•Participate as part of the Bridge Team with special events for participants.
•Attend required meetings and prepare all required reports as identified by the Bridge Administrator.
•Other appropriate Bridge-related duties as assigned by the Bridge Administrator.
