Inpatient/Outpatient Advanced Practice Pharmacist
QUALIFICATIONS REQUIRED:
You must be a citizen of the United States.In addition, applicants must meet all of the requirements for the position as stated in VA Handbook 5005, Part II, Appendix G15 and have full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United Sates or the District of Columbia.Minimal qualification requirements include the completion of the equivalent of one (1) year of professional pharmacist experience at the next lower grade level that has provided a broad knowledge of the complete pharmacy operation.Experience may have been gained as a clinical pharmacist practicing in a major specialty area or as a pharmacy specialist responsible for a major program area within a pharmacy service.Qualifying experience may also have been obtained as a pharmacy supervisor responsible for the professional and administrative management of an inpatient/outpatient area in a large pharmacy service or an inpatient/outpatient is at a site geographically removed from the supervisor.
Education completed in foreign colleges or universities may be used to meet the above requirements if you can show that the foreign education is comparable to that received in an accredited educational institution in the United States. It is your responsibility to provide such evidence when applying.
You must be a U.S. citizen to qualify for this position.
After appointment, you will be subject to random testing for illegal drug use.
You must have a certification, licensure, or registration credentials in License for this position. In some cases, these may be substituted for experience and education.
You will need to successfully complete a background security investigation before you can be appointed into this position.
Applicants for this position must pass a pre-employment medical examination.

The Advanced Practice Pharmacist is a staff member of the Pharmacy Service and functions as a team member on patient care team(s).Assists in developing staff and patient programs which promote optimal drug therapy.This may include review of patients on multiple medications (poly-pharmacy), pharmacokinetic dosage methods, drug therapy monitoring review, and other means to maximize safety and efficacy of drug use by impacting on the therapeutic decision making process.It may also include means of effecting reasonable patient understanding and compliance with drug therapy instructions and important untoward effect monitoring.Screens, profiles, and enters medication orders in the Computerized Patient Record System (CPRS). Has the responsibility to adequately research drug therapy questions and actively participate in providing advanced clinical pharmacist services requiring the highest levels of pharmacist clinical privileges.Participates in the recognition and documentation of adverse drug reactions, in projecting therapeutic objectives and in measuring success or failure toward the same; and in determining drug regimens to support optimal patient care. Participates in the professional and academic efforts of the Pharmacy Service, and acts as a resource in staff development and activities in clinical pharmacy practices.
