09-953(CS) Clinical Pharmacist QUALIFICATIONS REQUIRED: MEDICAL REQUIREMENT Applicants must be able to distinguish basic colors. This is a Drug Testing Designated Position. Selected candidate is subject to random drug testing Basic Requirements The licensed Clinical Pharmacist conducts pharmaceutical activities and services in an ambulatory care or internal medicine setting EDUCATION: 1. Graduate of a pharmacy degree program from an approved college or university.The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE) or prior to establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP). 2. Graduates of foreign pharmacy degree programs meet the education requirement if their degree is found to be equivalent to degree programs recognized the ACPE. This finding may be based on any of the following: A). A letter of acceptance into a U.S. graduate pharmacy program recognized by the ACPE. B). Written certification from the Foreign Pharmacy Graduate Examination Commission that the individual has successfully passed the foreign Pharmacy Graduate Examination. C). A letter from a U.S. college or university with a pharmacy degree program recognized by ACPE stating that the individual?s foreign pharmacy degree has been evaluated and found to be equivalent to its Bachelor of Pharmacy degree. LICENSURE:Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e. Puerto Rico) or the District of Columbia. EXPERIENCE: Completion of the equivalent of 1 year of creditable experience at the GS-11 grade level, which is directly related to the position to be filled. At least one year of professional practice in which the candidate demonstrated the following knowledge, skills, abilities and other characteristics. PREFERABLE EXPERIENCE:Completion of a post graduate residency in an ASHP accredited PGY 1 program. Demonstrated the following knowledge, skills, abilities and other characteristics.This position also includes anticoagulation pharmaceutical duties. You must be a U.S. citizen to qualify for this position. You must submit to a drug test and receive a negative drug test before you can be appointed into this position. You must have a certification, licensure, or registration credentials in Pharmacist for this position. In some cases, these may be substituted for experience and education. You will need to successfully complete a background security investigation before you can be appointed into this position. Applicants for this position must pass a pre-employment medical examination. 09-953(CS) Clinical Pharmacist Additional Duty Location Info:1 vacancy - Santa Maria, CA MAJOR DUTIES: BackToTop(); This position includes the following functions: Program Management: 1. Maintains good work flow patterns to keep outpatient prescription waiting time to an average of 20 minutes or less and ensures accurate dispensing and record keeping. 2. Effectively organizes workload, plans short and long range objectives, solves problems, and achieves goals within budget and staffing constraints. 3. Participates in quality assurance activities. Clinical: 1. Develops, plans, and implements innovative ambulatory care or internal medicine programs with an emphasis in total pharmaceutical care. 2. Evaluates a wide variety of patients in assigned patient care areas and makes appropriate recommendations on drug therapy.Designs, recommends, and implements pharmacotherapeutic regimens and corresponding monitoring plans. 3. Provides patient care utilizing an advanced in-depth knowledge of a specialized area of clinical pharmacy practice. 4. Responsible for patient care activities involving highly innovative methods of healthcare delivery.Includes clinical privileges in assigned area. 5. Provides clinical pharmacy consultations to professional staff on the pharmaceutics,pharmacokinetics, pharmacodynamics, and pharmacotherapeutics of pharmaceutical products.Must be competent to demonstrate the knowledge and skills necessary to provide care/services appropriate to the age of the patients served.Must demonstrate the knowledge and ability to assess data reflective of the patient?s status and interpret the information needed to identify each patient?s requirements relative to age-specific needs and to provide care needed as described in applicable policies and procedures. Administrative: 1. Responsible for the controlled substance accountability program and the drug accountability program for pharmacy. 2. Ensures assigned administrative projects and reports are submitted in a timely fashion. 3. Effectively utilizes the VISTA/CPRS computer system as well as automated pharmacy equipment. Education: 1. Serves as a preceptor to the ASHP pharmacy residency program and pharmacy students. 2. Collaborates with the Manager, Pharmacy Training and Development, to promote the learning experience and improve the performance of all levels of pharmacy staff. 3. Participates in all aspects of training such as conducting on-the-job training, conducting in-services, and monitoring performance. Research: 1. Works with researchers on protocol requirements and investigational record keeping.Assists in protocol development and implementation, as needed.
